Thank you for your interest in Health S.E.T. (Service, Empowerment, Transformation)

Health S.E.T. is a nonprofit organization founded by Catholic Sisters that provides free health care and social services to low-income elderly people in the Denver Metro Area.

Health S.E.T. has two main programs, both of which help low-income seniors to live independent, healthy lives.

Health Promotion Clinics – Volunteer and staff nurses provide monthly health screenings at 17 low-income high-rise residences in the Denver area. At least once each year, a volunteer pharmacist and a nutritionist attend each clinic to provide recommendations. The Health Promotion Clinics also address vision issues for seniors. The staff provides education, support, and access to devices (such as talking watches) that can facilitate daily tasks for low-income seniors residing in clinic buildings who are experiencing vision loss.  Services include eye chart exams; evaluation of the home’s safety and arrangement for ease of mobility; information about eye health; and access to adaptive aids.

Nurse Care Management – For people who have come to our Health Promotion Clinics but who have more complicated needs, we provide intensive case management. We assist people in this program with health concerns and with general social services.

We need your support. If you would like to make a financial contribution or if you would like to volunteer with us, please contact us at:
