HealthSET is dedicated to improving the lives of the low-income elderly. Our highly-skilled and compassionate nurses and case workers provide free health clinics and substantial follow-up care, acting as a bridge to the complex health care and social service systems. We give our most vulnerable seniors the tools to live independently which is key to preserving their quality of life.
A Client's Story
Joanne is a former nurse who has diabetes and has suffered multiple strokes. She now lives in poverty, and as a result of her strokes has great difficulty in communicating.
In order to manage her everyday tasks, she was paying a “friend” an outrageous amount for basic house cleaning and shopping. HealthSET learned that the amount charged ($900/month) was almost equal to the woman’s entire monthly income. A HealthSET Advocate helped Joanne break this arrangement and find affordable assistance.
Our staff is continuing to assist Joanne to make sure she receives the services she needs.

Through these comprehensive services, HealthSET increase the chances of early detection, mitigates significant health issues and connects clients with a medical home. HealthSET is a link in the continuum of care for the low-income elderly. For 25 years we have been promoting wellness so that our clients utilize the heath care system more effectively and at a lower cost to all.
Health Promotion ClinicsVolunteers and staff nurses provide free, essential, life-saving monthly health screenings for low-income elderly who reside in 20 rent-subsidized apartment buildings throughout metro Denver.
Geriatric Care Coordination
For our clients with more complex needs our Care Managers provide intensive case management, ensuring our clients receive necessary health care and social services so that they may continue to live independently, with dignity.
HealthSET is emphasizing proactive wellness through community Health Coaching. HealthSET staff and trained volunteers provide heath coaching to clients who have an unmanaged chronic condition and need support to become their own heath advocates. Ultimately, improving quality of life as they learn how to manage their condition so that they stay out of the Emergency Department and hospital and continue to live independently.
